Two things. First, the region has led California on harm reduction — naloxone distribution, syringe services, low-threshold buprenorphine — and that shapes what treatment looks like. Medication-assisted treatment is well established across the region and is the evidence-based standard for opioid use disorder, not a compromise.
Second, cost. Private programs here are among the most expensive in California, and the housing market pushes sober living rents well above what people expect. Kaiser covers a very large share of the Bay Area, and its structure is different from a typical PPO — care is generally coordinated within the Kaiser system, and going outside it usually requires authorisation.
